You must provide your Serial number/Civilian A-number, Rank/Title and a current City of Phoenix email address for verification, if Retired please provide your retired serial number as well as your personal email address.

 All sworn ranks and department-approved civilian positions are authorized to purchase these badges. Badges can be picked up at the museum upon receipt, mailed, or sent by interoffice delivery.
