You must be an active Phoenix Police Department employee and provide your serial number/civilian A-number, rank/title and a current City of Phoenix email address for verification.  All sworn ranks and department-approved civilian positions are authorized to purchase these badges (see department policy for authorized positions). Badges can be picked up at the museum upon receipt, mailed, or sent by interoffice delivery. *You will be notified when your badge arrives at the museum.
